This activity is intended for physicians, physician assistants, nurse practitioners, nurses, dietitians, pharmacists, certified diabetes educators and other interprofessional health care professionals.
At the end of this activity, the attendees should be able to:
- Identify patient characteristics to establish individual glycemic targets using a patient-centered approach as described in the American Diabetes Association’s (ADA’s) Standards of Medical Care in Diabetes - 2019
- Discuss how to use antihyperglycemic agents based on individual patients’ history and comorbidities to reach individualized A1C targets
- Evaluate the multiple treatment options available to providers when intensifying therapy
- Calculate and establish baseline insulin dosages and how to properly titrate dosage based on patients’ self-monitoring blood glucose
